The Program Coordinator I, under general supervision, coordinates the planning, development and implementation of the Quality Improvement Program. This role provides meticulous organization, coordination, and execution of all activities related to quality measures for our patients. Serves as liaison to clinical leadership and insurance partners. Performs data collection, analysis, reporting, and the implementation of targeted interventions to drive continuous improvement in patient outcomes.

Required Education and Experience:

  • Bachelor's degree or equivalent combination of education and experience

  • Two years of related experience in office administration, coordination, or other related job functions

Required Knowledge, Skills, And Abilities:

  • Ability to multitask and work cooperatively with others

  • Interpersonal and communication skills

  • Planning and organization skills

Preferred Qualifications:

  • Experience in working with Quality Initiative in patient care

  • Understanding of HEDIS Measures, Quality incentives, Care Gaps

Responsibilities:

Administrative

  • Compiles and reviews program statistics and data from insurance partners.

  • Distributes program data to Quality support staff to extract needed data information patient records to ensure measures are being met.

  • Collects program data from clinical reports for submission to insurance partners.

  • Maintains program records and databases.

Project Coordination

  • Coordinates of Quality Programs, including but not limited to HEDIS, Care Gaps, etc.

  • Initiates targeted interventions through data analysis to improve patient outcomes.

  • Compiles program statistics and Coordinates program communications and marketing.

  • Plans and implements program events such as conferences, seminars, lectures, meetings, and workshops.

  • Responds to inquiries regarding program offerings.

  • Plans and implements programs and assists in their evaluation.

Liaison

  • Acts as a liaison between clinical leadership and insurance partners. This will include attending regular meetings with insurance partners and sharing findings with clinic leadership .

  • May develop and update training materials and deliver training sessions.

Patient Outreach

  • Coordinates outreach to patients for non-compliant patients.

  • Performs other duties as assigned.
