Job Description Park Nicollet Job Title: Nurse, Pract Sen Health Job Code: 2PX-0031 Leader Title Level: Manager Location: Varies Department: Community Senior Care Final Approval (Dir. or VP Level) Director Date Approved: 3/12/2020 Date Revised: 7/5/2023 POSITION SUMMARY STATEMENT: This clinical professional provides quality, evidence-based patient care within their scope of practice in their assigned care settings and provides care coordination across the continuum. The individual maintains high standards of care and service in accordance with the Nurse Practice Act and PNHS. The NP supports improvement of existing processes and outcomes; including but not limited to patient health outcomes, patient and family experience, and confidentiality. MINIMUM QUALIFICATIONS: Education, Experience or Equivalent Combination: Master's degree or DNP is required. Licensure/ Registration/ Certification: Must be a registered nurse licensed in the state of Minnesota, who has successfully completed a formal nurse practitioner program approved by a national accrediting agency and who is then certified by a national certified organization and licensed by the Minnesota State Board of Nursing as an APRN. Knowledge, Skills, and Abilities: Proven proficiency with: Various health testing devices, Blood drawing or injection devices, Computer skills, Electronic medical record, Current patient care technology, Office equipment (telephone, fax, communication systems) Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients. Ability to effectively problem solve and make sound decisions. Essential Duties: % Of Time (minimum 5% increments) 1. Provides onsite medical visits: involving observation, assessment, planning, implementation, and evaluation. 40-80% 2. Offers preventative health care counseling and instructs patients on treatment plans. 10-30% 3. Orders, interprets, and evaluates diagnostic tests to identify and assess patients' clinical problems and health care needs 1-25% 4. Performs therapeutic and diagnostic procedures as indicated by training and experience. 1-30% 5. Consults with other members of the health care team on difficult, unusual, or abnormal cases and makes appropriate referrals and consultations for evaluation or treatment. 5-25% 6. Makes nursing facility rounds as required by CMS or patient condition. 0-40% 7. Assists in the development of new policies, programs, procedures, materials, or techniques for designated department. 1-10% 8. Serves as a resource to nursing staff, and other support staff, providing education. 1-10% 9. Performs patient follow-up including returning phone calls, evaluating test results, completing documentation and necessary paperwork. 5-30% 10. Utilizes principles of quality of life, maintenance of optimal function, shared decision making and patients' advance directives in developing the plan of care. 100% 11.
