Primary Function/General Purpose of Position
The Bariatric and Orthopedic Program Coordinator is responsible for the overall administration and operations of the Bariatric and Orthopedic Program. The Bariatric and Orthopedic Program Coordinator in collaboration with the unit leaders is responsible for integration, facilitation and coordination of services for this patient population. Responsible for development and implementation of initiatives to positively impact quality patient care as evidenced by improved patient outcomes measured by Centers for Medicare and Medicaid Services (CMS), Health Grades, and The Joint Commission. Scope of patients includes all Bariatric Surgical Patients, Total Joint Replacement (TJR) elective, revisions & fractures including knee, hip, shoulder, elbow and ankle, spine, foot/ankle and sports orthopedic patients.
Essential Job Functions
Develops strategies in conjunction with Nursing Directors, Operations Directors, Surgical Services, Dietary, Administration and Providers, to achieve accreditations pertinent to bariatric and joint and spine orthopedic programs
Develops plans in conjunction with Nursing Directors, Operations Directors, Surgical Services, Dietary, Administration and Providers, for future Bariatric and Orthopedic services for expansion and changes in existing services that are consistent with organizational strategic plan
Participates in development of long range plans for growth of the bariatric and orthopedic service line
Participates in development of long range plans for growth of the bariatric and orthopedic service line
Assists with development and monitoring of program budget
Reviews all data and findings to understand and resolve operational issues which impede program goals
Establishes and maintains project plans and timelines
Reviews and analyzes monthly reports. Determines if trends exist and intervene appropriately
Evaluates 100% of bariatric and joint replacement and spine procedure charts for variances, and compliance with documentation guidelines. Develops action plans for required corrective action
Initiates processes for clinical and quality improvement, in coordination with Hospital Quality Improvement related to Bariatric and Orthopedic services
Develops and implements performance improvement processes focused on indicators at variance
Provides reports with recommendations in the Bariatric and Orthopedic Committees.Follows-up for improved compliance
Initiates processes in coordination with Hospital Quality Improvement for clinical and quality improvement related to Bariatric and Orthopedic services. Develops and implements performance improvement processes focused on indicators at variance
Analyzes patient satisfaction data specific to bariatric and orthopedic patients and implements strategies in conjunction with the Nursing Leaders to reach top 90th percentile
Develops processes and polices related to the Bariatric and Orthopedic programs in coordination with leadership team
Facilitates the Multidisciplinary team meetings and committees.Ensures timeline and key deliverables are achieved
Provides necessary staff education in collaboration with Nurse Leaders to ensure expected outcomes. Teaches and precepts appropriate staff to teach pre-operative classes and assesses impact of education on outcomes
Works with physician groups and Public Relations department to develop community bariatric and orthopedic seminars. Develops, revises and implements all patient education materials and notebooks.Ensures materials are standard and consistent with standard order sets and clinical pathways.Works with physicians and staff to update on an annual basis or as needed to maintain high quality of care standards
Stays current with trends in Bariatric and Orthopedic services
Serves as a primary point of contact for bariatric and orthopedic/spine patients through the course of pre-admission, acute care and treatment and recovery 30-90 days post-surgery. Manages post op patient phone call communication
Functions as a member of an integrated professional team in the ongoing accurate assessment of patient needs through rounding with physicians and nurse caregivers to promote the development, implementation and evaluation of the nursing care plan on an ongoing basis
Demonstrates excellence in service behaviors to increase patient satisfaction scores
Participates in an employee engagement plan for the department to improve work environment.Communicates department/corporate goals to associated staff in order to facilitate team unity and motivate individuals
Serves as resource to associated staff.Responds to questions and concerns appropriately and in a timely manner. Provides method of obtaining continuing education as needed.Provides back up support for difficult projects or assignments. Serves as liaison between staff and senior management. Provides instruction, encouragement and support as needed.
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.Provides patient care to neonate, pediatrics, adolescents, adults, and geriatrics.Coordinates with integrated team, plan for special needs patients
Manages patient engagement platform and appropriate communication
Participates, supports and facilitates ad hoc committees i.e. process improvement.Performs other duties as assigned. Obtains reports, analyzes data, enters and reconciles charges as assigned
