The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.
Position Summary
Candidate with a Licensed Master of Social Work (LMSW) is sought for a part-time position (8-10 hours a week) within the Department of Neurology's Muscular Dystrophy Association Clinic.
Responsibilities
- Provide social work support for pediatric patients and adult patients who are cared for by the Muscular Dystrophy Association Clinic
- Complete forms required for referrals including homecare, in-home rehab services, respite, transportation, palliative care and hospice services, M11Q forms, durable equipment forms, and any other forms that can be completed within the social worker's scope of practice.
- Compose letters of medical necessity.
- Provide case management, crisis intervention, and supportive counseling services to the individuals and families seen within the MDA clinic.
- Provide referrals and resources to patients and caregivers.
- Participates in staff meetings, in-services, administrative activities, and community outreach events.
Minimum Qualifications
- Must hold a master's degree in social work, and 2 years of related experience.
- Must hold a New York State LMSW or LCSW license.
- Certificate from the New York State Education Department.
Preferred Qualifications
- Bilingual in English/Spanish strongly preferred.
Previous experience working with this population, or as a medical social worker.
