Columbia University Triage Nurse in New York, New York

Position Summary

The Triage Nurse (Team Lead) collaborates in a team based care environment providing professional appropriate nursing services to patients and/or family members both via telehealth and face to face.This is done utilizing a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act and its recognized professional standards. The Triage Nurse will function as a liaison between physicians, patients, and staff in the delivery of complex telephone care and coordination. The individual must be flexible to prioritize patient and team needs both on the phone and in clinic with strong interpersonal and communication skills. The nurse must be a patient advocate, maintaining the patient’s confidentiality and patient rights. The Triage Nurse must provide assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act and its recognized professional standards. The nurse resolves patient concerns or complaints while maintaining a positive personality and friendly attitude, addressing patient needs in a timely and appropriate manner. Functions as a mentor to other team members and works collaboratively with team members to identify and resolve department and/or guideline issues through established procedures. Supports a team environment which allows clear communication, positive support and creative thinking.


Tasks include, but are not limited to: coordinating patient care; patient education; developing a nurse-patient/family relationship; and assessing and managing patients within all age groups from pediatrics to geriatrics. Perform diagnostic testing, analysis of results, and communication of results to the patient. The incumbent needs to be a meticulous recorder of patient information. Therefore, excellent interpersonal communication skills are a must, along with the ability to extract information during patient history interviews. The nurse must be able to carefully and easily explain a treatment and follow-up plan to patients.

This nurse will provide daily informal leadership to clinical support staff (CMA’s, front desk), delegating as appropriate to deliver timely and high quality care with excellent customer service.

Additionally, the incumbent will contribute to the patient experience and daily work flow of the clinic by performing the following duties:

  • Nurse Triage: Answer and triage incoming patient calls, document appropriately in the medical record, respond to messages from clinicians and other providers.

  • Direct Patient Care: Assess patients using all available physical and diagnostic data, incorporating medical history and evaluate patient status and progress with current plan of care. Assist Physicians with assessment, evaluation and management of patient care of patients.

  • Education and Care Coordination: Provide education to patients and families on what to expect during office visits and procedures, and provide specific pre- and post-procedure instructions. Perform patient and family education and counseling including disease state, management/plan of care, side effect, treatment, medications, all follow up

Medication Management: Medication refills per department protocol. Prepare, dilute, draw up medication as needed for use in the faculty practice and Resident clinic. Observes, records, and reports patient's condition and reaction to drugs and treatments to physicians. Dispenses medication as directed

  • Prior Authorizations: Manage and facilitate obtaining prior authorizations as needed.

  • Assist walk-in patients by answering questions and assisting patients per clinic protocol

Informal Leadership: provide informal leadership to the clinical care team, delegate as appropriate, identify ways to improve patient care and experience. Provide solutions for complex problems.

  • Maintain care environment: Maintains exam rooms for necessary supplies and materials. Ensures cleanliness. Provides support when preparing list of supplies needed. Prepares equipment and aids physician during treatment, examination, and testing of patients.

  • Attend staff meetings, patient support meeting, and continuing education pertaining to specialty as appropriate

  • Participate in quality improvement initiatives, department in-services and workshops, and other educational opportunities to enhance the patient experience

  • Ensure compliance with applicable regulatory agency standards (e.g., OSHA, JCAHO, DOH, HIPAA, billing compliance, etc.)

  • Provide backup coverage to both floor support staff, rooming and discharging patients as needed and support for Dermatology medical procedures (including phototherapy). Provide cross coverage and other duties in other areas as the need arises.

Minimum Qualifications

  • Candidate must be a Registered Nurse or Licensed Practical Nurse with a degree from an accredited nursing program. This candidate must be licensed (or eligible) to practice In New York State. Previous dermatology experience, phone triage, and / or leadership experience, preferred—preferably in an academic ambulatory care setting. BLS certification required.

  • Strong critical thinking and problem solving skills and ability to learn information in a fast-paced environment. Excellent clinical and human relations skills. Ability to effectively communicate both orally and in writing.

  • Knowledge of medical safety practices and ability to evaluate existing standards and implement new policies and procedures; knowledge of current clinical best practices and clinical state-of-the-art procedures; and knowledge of organizational policies, regulations and procedures to administer patient care.

  • Excellent oral/written communication , organizational, and computer skills (Allscripts electronic medical record, Microsoft Office).

  • Ability to work effectively both independently and within diverse workgroups and patient population.

  • Ability to identify problems, research and recommend solutions, develop and maintain quality systems, and exercise a high degree of initiative, judgment, discretion and independent decision-making.
