Richmond Community Services Community Coordinated Supports Mentor in Mount Kisco, New York

Richmond Community Services is currently seeking ComHab/DSPs/Community Coordinated Support Mentor (CCSM) for our innovative ComHab programs throughout Westchester County.

Community Habilitation offers a wealth of valuable, one–on–one services to children and adults with disabilities. The program offers choices and increased independence while encouraging productivity and shared community involvement. Through a variety of activities geared toward enhancing daily life-skills development, our knowledgeable, creative staff provide families with much-needed assistance by spending time with program participants in their homes and out in the community.

Independent self-starters needed to work directly one-on-one in the community with individuals with intellectual disabilities. Responsibilities include training individuals in the performance of personal hygiene and self-care tasks (dressing, grooming, etc.) general household chores, mobility to maximize independence in personal health care and travel, financial management, appropriate social behaviors. Duties include: following the Staff Action Plan based a person - centered approach, recording and monitoring progress, maintaining appropriate documentation, and assume driving responsibilities.

The ComHab/DSP/CCSM works one-on-one with individuals with intellectual and developmental disabilities by using strategies and community-based activities designed to promote skill building in the areas of communication, socialization, behavior, independent living, movement, etc. CHDSP's are involved in the creation and assessment of habilitative and behavioral plans that they will implement.

The CHDSP demonstrates knowledge of Richmond’s quality mission, vision, and values. In addition, this position actively participates in quality improvement projects (i.e. communicating issues, offering suggestions) to ensure ongoing excellence of Richmond residents, staff, and programs.

Requirements:

  • Associates Degree/Human Services Degree or experience preferred

  • Minimum of 2 years of experience working directly with Individuals with developmental disabilities

  • Must have valid drivers license for at least 3 years and successfully complete and maintain Richmond Driver Authorization - NO EXCEPTIONS

  • Must have reliable transportation and must maintain valid motor vehicle insurance

Salary - based on experience between $20 - $23.09/hour
