HCR Home Care Physical Therapist - Fulltime in Malone, New York

Role and Responsibilities

A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home following physician’s orders.


Essential Functions

  • Evaluate patient using diagnostic muscle, nerve, joint and functional ability tests and measures.

  • Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.

  • Recommends to physician, modification of plan of care as needed.

  • Documents clinical notes on the date of the visit into medical record and keeps all records and summaries up to date.

  • Treat patients to address impairments and functional limitations to reach maximum functional independence.

  • Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic and durable medical equipment (canes, walkers, wheelchairs, etc.)

  • Include and educate the caregivers and patient regarding treatment plan and goals.

  • Attend physical therapy staff meetings and in-service presentations.

  • Participate in case conferences.

  • Participate in the development and periodic revision of the physician’s Plan of Treatment.

  • Assures appropriate safety practices and maintenance of equipment.

  • Other duties as assigned.


Education Requirements

  • Graduate of an accredited physical therapy program.

Qualifications and Requirements

  • Licensed by and currently registered with the New York State Education Department.

  • One year of practical experience as a licensed physical therapist.


Physical Demands / Work Environment

The physical demands described below must be met by an employee to successfully perform the essential functions of this job, and to ensure client safety at all times in the household. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • The employee must be able to hear within normal range with corrected hearing in a minimum of one ear, and have corrected vision of 20/40 bilaterally.

  • Frequent walking and standing (80%-100% of the time)

  • Frequent lifting

  • Must be able to lift / move 40 lbs. unassisted

  • May require pushing / pulling / transferring 100 lbs. of weight

  • Sufficient hand strength and dexterity to carry out care plan duties

  • Ability to bend, stoop, reach, pull and push unsupported, routinely.

  • Ability to climb two flights of stairs.

  • Must be able to sustain prolonged periods in seated position.

  • Working conditions may expose the care giver to smoke, pets and other allergens.

  • Potentially hazardous materials such as those found in cleaning agents and other chemicals may be stored in a client’s home.
