JOURNEY MENTAL HEALTH CENTER INC Behavioral Health Care Coordinator in MADISON, Wisconsin

JOB REQUIREMENTS: Come with work a cohesive, supportive team in the CCS GROW program! The Behavioral Health Care Coordinator position works within the CCS program for Dane County, providing robust and comprehensive care for people with mental health and/or substance use recovery. This includes community based direct services with clients and their systems, creating individually tailored recovery plans, assessments, and services to fit their needs. Typical Physical Demands - Requires driving, sitting, standing, light lifting, bending, and reaching; and the ability to use and operate standard office equipment, including computer, calculator, phone, printer, cell phone, facsimile, etc. - This position requires computer work and at times sitting at a desk for two or more hours at a time. Working Conditions: -May require evening hours depending on program and consumer\'s needs. -Work is performed in a busy office environment and in the community serving consumers of all ages. - A significant amount of consumer contact occurs in the community and at times within consumers\' homes. - A limited amount of work may be done via telehealth dependent on program needs. PREFERRED: - Bi-lingual, and/or bi-cultural (Spanish/Hmong/Cambodian). - Fluent in American Sign Language. - Experience with use of evidence-based practices in mental health, and substance abuse. Encourage clients to express their feelings and discuss what is happening in their lives, helping them to develop insight into themselves or their relationships. Maintain confidentiality of records relating to clients\' treatment. Assess patients for risk of suicide attempts. Prepare and maintain all required treatment records and reports. Guide clients in the development of skills or strategies for dealing with their problems. Fill out and maintain client-related paperwork, including federal- and state-mandated forms, client diagnostic records, and progress notes. Develop and implement treatment plans based on clinical experience and knowledge. Collect information about clients through interviews, observation, or tests. Discuss with individual patients their plans for life after leaving therapy. Modify treatment activities or approaches as needed to comply with changes in clients\' status. Evaluate clients\' physical or mental condition, based on review of client information. Collaborate with mental health professionals and other staff members to perform clinical assessments or develop treatment plans. Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations. Coordinate or direct employee workshops, courses, or training about mental health issues. Plan or conduct programs to prevent substance abuse or improve community health or counseling services. Gather information about community mental health needs or resources that could be used in conjunction with therapy. Learn about new developments in counseling by reading professional literature, attending courses and seminars, or establishing and maintaining contact with other social service agencies. Refer patients, clients, or family members to community resources or to specialists as necessary. ***** OTHER EXPERIENCE AND QUALIFICATIONS: Experience: - Experience in related behavioral health field. -Commitment to work toward improving cultural diversity and inclusion. -Strong commitment to principles of recovery and to facilitating consumer participation in treatment/service planning and provision. - Knowledge of evidence-based practices for treatment of mental health and co-occurring disorders. -Experience in community-based mental health and substance abuse assessment, case management, treatment planning and crisis resolution. - Ability to communicate effectively (both orally and in writing). - Ability to work effectively with an interdisciplinary team with a positive and creative attitude. - Ability to work independentl with highly developed organizational skills and ability to multitask. - Superior ethics and interpersonal boundaries as evidenced by experience in community-based work with related professional references. ***** APPLICATION INSTRUCTIONS: Apply Online: www.journeymhc.org/join-us/join-our-team
