Faculty physicians will practice primary care, non-surgical obstetrics, and facilitate a rotation site for third- and fourth-year medical students. DFCM has additional primary care practices in Lexington, Georgetown, Hazard, and Hindman, Kentucky.With several clinics in the state, we serve as leaders at UK HealthCare, a $4 billion health system, in its primary care delivery strategy by implementing value-based clinical redesign and population medicine programs. We have aligned our clinical approach to enhance value-based quality measures through pre-visit planning, comprehensive nurse assessment, and a UK HealthCare Population Health team involving social workers, community health workers, and RN care managers. We are a national leader in transitional care management and its impact on unnecessary hospital readmissions.
