Job Summary and Qualifications

The Sepsis Coordinator facilitates the development, implementation and ongoing adherence of sepsis program protocols and therapies. Ensures the protocols and therapies become the standard of care followed within the facility. Collaborates with medical and hospital staff to provide multidisciplinary care to patients with a diagnosis of sepsis. Rounds with physicians on patients with a diagnosis of sepsis and reviews clinical data. Provides education and clinical expertise to care team, patients and families. Collaborates with physicians and staff in an effort to decrease mortality and length of stay. Identifies and integrates patient learning needs, abilities and readiness into the Plan of Care. Works in collaboration with the nursing team to develop and implement individualized teaching plans for patients and families regarding sepsis diagnosis. Collects and analyzes sepsis program patient data to assist with documentation, clinical care and improved outcomes. Participates in rounds and proactively responds to patient needs to improve patient outcomes and positively impact overall patient experience.

Participates in Performance Improvement activities to assure continuous Quality Improvement as appropriate. Maintains cultural and diversity awareness when dealing with patients, visitors, and other customers. Promotes a supportive, team-work oriented environment. Fulfills departmental responsibilities with the long-range plan, mission, vision, and values at Horizon.

Essential Job Duties & Responsibilities

1. Reviews all positive screens on a daily basis and collaborates with primary nurse and physician. Able to navigate Meditech with confidence and retrieve patient care information.

2. Demonstrates responsibility and accountability to assure early goal-directed therapy is initiated.

3. Provides leadership to promote teamwork; work with multidisciplinary groups and facilitate problem-solving tasks, using critical thinking techniques.

4. Initiates and guides staff in appropriate interventions associated with sepsis protocols.

5. Able to articulate the NPSG meaning and measuring process; uses critical thinking and effective communication with other healthcare providers and departments.

6. Provides regular in-services to nursing staff (including on sepsis screening and documentation) and ensures understanding. Participates in hospital orientations as needed regarding sepsis education.

7. Collects and presents abstracted data for analysis purposes. Organizes sepsis meetings to assure dissemination of relevant information. Attends division meetings and other activities as required.

8. Communicates on a daily basis to answer and/or educate nursing staff. Responsible to stay abreast of teamroom updates and files as required.

9. Rounds with physicians as necessary on sepsis patients; provides guidance as required.

10. Identifies role as a team member and works efficiently to achieve goals of team, department and hospital. Demonstrates professional behavior in actions and consistently portrays a positive attitude in response to work changes.

11. Demonstrates to patients, family members, physicians and co-workers courtesy and caring through excellent customer service.

12. Demonstrates ability to communicate clearly, directly and professionally. Listens and responds with empathy.

13. Demonstrates excellent customer service skills; acknowledging others, phone skills, anticipating and responding to customer needs.

14. Actively attends all staff meetings and keeps current on departmental communications and information including, but not limited to, patient safety and quality improvements.

15. Timely completion/submittal as per hospital policy for Licensure, Certifications, annual TB and department-specific competencies as required by job description.

16. Follows the policies for attendance, punctuality, time-keeping, meal breaks, usage of cell phones, dress code and confidentiality of information.

17. Demonstrates an understanding of and adherence of the Code of Conduct. Behavior reflects Horizon’s values and a commitment to the Code of Conduct.

Qualifications

· Current licensure with the TN Board of Nursing;

· BSN required; Master’s preferred

· 3-5 years of prior clinical experience is preferred

· Experience on personal computers using spreadsheets and word processing

· BCLS required

· ACLS required
