Oswego Industries located in Fulton NY is hiring a full time Plan Coordinator.

Responsibilities include -Attend Participant annual and semi-annual individualized planning meetings -Develops, implements, and uploads, participant plans and objectives with regulatory timeframes. -Documents individual safeguards and update as necessary. -Responsible for collection and distribution of plans and eligibility information. -Collects and maintains attendance data per agency policy and billing standards. -Train Direct Support Professionals on updates, and/or changes to programs and safeguards. -Provides direct care back-up coverage in all Habilitation Programs as needed. -Write program notes for assigned individuals supported. -Review and quality control of daily billing documentation as assigned. -Incorporate choice and informed decision making into all phases of planning. -Assess strengths and barriers to individuals' success and develop strategies to promote success. -Collaborate with Nursing and client team as participants' needs require. -Provide support to programs staff in implementing goals and programs in site-based and community settings. -Work as part of a cohesive interdisciplinary team to support participants' engagement in programs and goal achievement. -Augment current job knowledge by participating in conferences, seminars, workshops, etc., conducted both with the agency and outside agency. -Maintains confidentiality of all persons served and contributes to the improvement of agency operations. -Acts in accordance with policies, procedures and the Agency Corporate Compliance Policy. Reports suspected or actual violations thereof to the appropriate party.

Education -Associates Degree in Human Services or a related field of study with 1-3 years of experience working in a disability services agency or similar program. -High School diploma or equivalent and combination of education, training and 2-5 years of experience working with people with disabilities.

Requirements A valid, clean NY State drivers license

Schedule -Day shift -Monday - Friday

Pay Rate $17.00- $22.10
