PLEASE REVIEW THE FULL JOB DESCRIPTION AThttps://www.philipstownhub.org/careersThe COMMUNITY & CRISIS CARE COORDINATOR (“CCC”) is a new position at The Hub, reporting to the Executive Director and working with other Care Coordinators. The CCC is responsible for the management of assistance given to client-participants and matters pertaining to that assistance. This includes (a) direct support for the work of each care coordinator; (b) being available for consultation with care coordinators in real time; and (c) taking an active collaborative role in cases with some complexity or urgency. This also includes (a) ensuring positive and effective working relationships with referral entities by knowing their range of services and their expectations regarding the referral process; (b) monitoring and facilitating discharge planning and after-care for participants, as appropriate or necessary; and (c) ensuring the timeliness of The Hub’s follow-through. The CCC is responsible for facilitating effective client-participant data collection, monitoring/reviewing the integrity of this data, and generating reports as needed. This includes keeping the Executive Director apprised of all current cases in the manner and level of detail requested, as well as ensuring that difficult situations are communicated in real time to the Executive Director. This also includes participation in case conferences, training, workshops and seminars, as well as meetings with peers as appropriate.The CCC is responsible for initiating, executing and/or sustaining organized activities and programs involving client-participants, community members and community-based organizations and/or institutions. This includes (a) the generation of ideas for additional services and/or modifications to our existing services and systems; and (b) suggesting best practices regarding consumer satisfaction and client outcomes.The CCC is responsible for researching and generating material to be used in grant proposals relevant to client-participant care and/or Hub programs, activities and events.

Minimum Salary: 55000 Maximum Salary: 55000 Salary Unit: Yearly
