About the Department

The Comprehensive Care Program (CCP) is located in the Section of Hospital Medicine, Department of Medicine at the University of Chicago. The program, originating as a study from the Center for Medicare and Medicaid Innovation to test whether physicians can provide better care at lower costs than the current system, allows hospital physicians to see their patients in the clinic as well as in the hospital, believing that patients will get better care, have better care at lower health care costs and be happier with their care if the same doctor is seeing them in both settings. CCP comprises physicians, study investigators and research team, and a clinical staff that includes nurses and social workers serving approximately 1,000 patients, and includes a provision for Home Care (CCP-HC) in which a physician and social worker team treat patients in their homes who cannot travel to clinic. While still under evaluation, CCP is viewed as a novel model of health care delivery and under consideration for replication in other healthcare systems.

Job Summary

The CCP Social Worker is a member of the Comprehensive Care Program\'s (CCP) clinical team, specifically focused on providing direct care to patients enrolled in the Comprehensive Care Program. This role will also support research initiatives and will be responsible for collaboration with and facilitation of content in a community-based cultural and arts program.

Responsibilities

• Completes comprehensive psychosocial assessments, provides support and interventions for a panel of chronically ill patients with complex social needs.
• Meets directly with patients and their family members in order to address unmet social needs such as but not limited to housing, food insecurity, social engagement, public benefits, and insurance navigation.
• Knowledgeable on community resources that can provide appropriate interventions and/or support services to meet patient needs.
• Initiates and maintains professional working relationships with community partners to advocate for appropriate and timely referrals.
• Facilitates individual, family, and group meetings and/or therapy sessions.
• Acts as a collaborative partner with members of the inpatient healthcare team to facilitate appropriate transitional care plans and care coordination/transitions efforts. Continues to provide clinical case management to patients once they return to the community.
• Collaborates closely with inter-professional team, attends daily multidisciplinary rounds and actively participates in daily case planning with a community health worker to coordinate care and address unmet social needs.
• Continuously assesses patient readiness and motivation in patient-identified goals and provides behavioral case management.
• Assesses for depression, anxiety, suicide ideation and substance abuse using validated screening tools.
• Provides brief crisis intervention to patients experiencing intimate-partner violence, sexual assault, and abuse or neglect.
• Understands and abides by mandated reporting policies and facilitates the involvement of Illinois Department of Children and Family Services or Illinois Department of Aging when there is concern for suspected abuse or neglect.
• Participates in program process improvement, community/wellness events, and research activities.
• Documents patient encounters in electronic medical record in a timely manner.
• Plans own resources to combat social problems, prevent substance abuse, or improve community health and counseling services.
• Analyzes potent al problems in home or work environments of clients.
• Performs other related work as needed.

Minimum Qualifications

Education:

Minimum requirements include a college or university degree in related field.

Preferred Qualifications

Experience:

• Post-graduate clinical experience in a medical setting, with a solid understanding of the medical system, medical terminology, and levels of care.
• Familiarity and experience with motivational interviewing, dialectical behavioral therapy, acceptance and commitment therapy and other evidence based practices.

Licenses and Certifications:

• LSW strongly preferred within 3 months of start date.

Technical Skills or Knowledge:

• Knowledge of federal and state laws, rules, regulations, NASW Code of Ethics, policies, and procedures as related to social work.

Preferred Competencies

Comfortable establishing an active role on an inter-professional team.
